Physical Examination

Students registering for Athletics must have a current physical examination on file in the Activities office before the student is authorized to participate in practice. To ensure clearance for participation, signed physical forms should be completed and uploaded, to the student account or a picture of the signed physical card can be emailed to Dave Gerber at dgerber@cameron.k12.wi.us in to the Athletics Office, prior to the first day of competition.

Physicals are valid for 2 year from the date of the last exam.
